Please print, read, and complete all the Forms below for Enrollment. Bring these with you on the first day of class. Your child cannot be considered for a part until ALL the forms below are completed and submitted. If child is an extra or on the tech crew, complete the Extras TechTuition form above, too.
** Please let Annie know if you cannot attend a practice or will be late. Be prepared and ready to have fun, learn, make friends,
and rehearse during play practices.
** Tuition is due the first class of the month. A late fee of $10 is charged after the second class of the month.
